Only customers who have completed and signed a Practitioner Directory form will have their information shared and we will only share the information you have provided on the form (Name, address, phone number, clinic name, practice focus). This information is for patient referrals only. We do not share or sell this information with any third parties companies.

Byron White Formulas, Inc. invites visitors on its site to send email messages so that we may respond to inquiries regarding our products and services. Byron White Formulas, Inc. retains copies of these email messages and content to respond to the request and to satisfy any legitimate interest in our products. The information provided by a visitor will be used to respond to the visitor’s inquiry and to provide the best possible customer service.

Customers

For customers using our online shopping cart, powered by 3dCart, Byron White Formulas, Inc. collects personal information necessary to provide our services. This information includes the customer’s name, company name, email, billing and shipping addresses, phone number and tax id. Byron White Formulas, Inc. uses this information to fulfill services of processing orders and maintaining account information and order history. Byron White Formulas, Inc. does not keep credit card information on file. Transactions are processed through Authorize.net. By ordering through our online store you are submitting your name, billing address and payment information. Once an order is processed we do not have access to the full payment information, all that is visible is the last four digits of your payment information and transaction information correlating to your purchase order. By submitting your order you are agreeing that this information be shared in order to process and complete your order. Byron White Formulas, Inc. processes shipment to customers using Shipworks, Stamps.com and when specially requested UPS and DHL. By submitting your order through our website we use your shipping address information through one of these services in order to process and send your orders. The only information submitted through these service providers are your name, mailing address, phone number and/or email.
